NEW DELHI — In a major breakthrough for stroke rehabilitation in South Asia, researchers from the Indian Institute of Technology (IIT) Delhi, in collaboration with the All India Institute of Medical Sciences (AIIMS) New Delhi and the Institute of Human Behaviour and Allied Sciences (IHBAS), have developed the first Hindi version of Melodic Intonation Therapy (MIT). This music-based intervention is specifically designed to help stroke survivors regain their ability to speak.

The landmark pilot study, published in the peer-reviewed international journal Aphasiology in June 2026, bridges a critical gap in neuro-rehabilitation. By adapting a decades-old musical therapy to the linguistic nuances of Hindi, the research offers a promising, culturally tailored lifeline to hundreds of millions of native speakers across India and the global diaspora.

Understanding Post-Stroke Aphasia

Every year, millions of people worldwide suffer a stroke. One of its most devastating yet under-recognized consequences is aphasia — a neurological communication disorder caused by damage to the brain’s language centers, typically located in the left hemisphere.

According to clinical data from AIIMS New Delhi, aphasia affects approximately 21% to 38% of acute stroke patients. While these individuals often retain full cognitive awareness and can understand spoken words, the neural connections required to articulate their own thoughts are severely disrupted.

Interestingly, clinicians have long observed a striking paradox in neurology: many stroke survivors who are entirely unable to speak standard sentences can still sing full lyrics to familiar songs without hesitation. This phenomenon occurs because singing engages preserved, multi-sensory neural pathways across both hemispheres of the brain, particularly in the right hemisphere.

The Science: How Music Rewires the Injured Brain

Melodic Intonation Therapy (MIT) exploits this exact phenomenon. By pairing spoken phrases with simple melodic pitches, rhythmic tapping, and steady repetition, MIT helps the brain bypass damaged speech centers and build new pathways for communication.

“Music therapy taps into brain networks that survive a stroke even when speech or movement pathways are damaged,” explains Dr. Sudhir Kumar, a neurologist at Apollo Hospitals, Hyderabad, who was not involved in the research team. “Rhythm helps retrain the brain’s motor circuits. Patients with gait difficulties often walk more fluidly when moving to a steady beat — a technique we call rhythmic auditory stimulation.”

This structural adaptation relies on neuroplasticity — the brain’s remarkable ability to reorganize itself by forming new neural connections throughout recovery.

Dr. K. K. Jindal, Director of Neurology at CK Birla Hospital, Delhi, notes that music acts as a powerful neurological catalyst. “Music, and in particular rhythm and melody, can cause several areas of the brain to be activated at once,” Dr. Jindal notes. For stroke survivors struggling with aphasia, channeling speech through melodic cadence allows the right side of the brain to step in and assist the damaged left side.

Bridging the Linguistic Divide

While MIT has been a staple of speech-language pathology in English and Western languages since the 1970s, applying English-based musical protocols to non-English speakers yields poor clinical results.

Languages differ significantly in their pitch variations, stress patterns, and rhythmic cadences. Hindi — spoken by more than 600 million people globally — possesses distinct phonetic dynamics that English-based modules simply cannot address.

To solve this, the research team — co-led by Prof. N.M. Anoop Krishnan and Prof. Rahul Garg from IIT Delhi, alongside Prof. Deepti Vibha from the Department of Neurology at AIIMS — spent nearly two years engineering a ground-up Hindi adaptation. Joint first authors Dushyant Soni and Rupanshi Ahuja meticulously restructured words, pitch contours, and rhythmic tapping patterns to mirror everyday Hindi conversation.

“In Indian music, there is no structured speech therapy module,” says Prof. Vibha. “So we are developing it and testing how effective it is compared to conventional therapies provided by speech therapists.”

Pilot Study Findings: Clear Clinical Progress

The initial feasibility study, conducted between December 2023 and March 2024, evaluated six Hindi-speaking stroke survivors diagnosed with non-fluent aphasia. Each participant completed 40 intensive sessions, each lasting 45 to 60 minutes, led by certified speech-language therapists.

The quantitative and qualitative outcomes were notably positive across all subjects:

Study Limitations and Future Research

While these findings represent a significant milestone, the authors caution that the preliminary data must be interpreted carefully due to the small cohort size.

“With only six patients, the results should be seen as a strong preliminary signal rather than final proof,” says co-lead researcher Prof. N.M. Anoop Krishnan. “Further studies with larger groups will be required to evaluate the effectiveness of the Hindi MIT approach.”

To validate these early results, the interdisciplinary team is preparing a larger randomized controlled trial (RCT). The upcoming trial is slated to evaluate two parallel groups of 30 patients, directly comparing the novel Hindi MIT framework against standard speech-language therapy.

Additionally, the team is exploring digital health solutions, including mobile applications and telehealth platforms, to bring this treatment directly to rural and underserved regions where specialist speech therapists remain scarce.

Public Health Implications and Practical Takeaways

Globally, stroke remains the second leading cause of death and the third leading cause of combined death and disability. In developing health systems, long-term rehabilitation for cognitive and language deficits is frequently inaccessible or cost-prohibitive.

The introduction of an evidence-based, culturally adapted speech intervention addresses an urgent public health need. However, experts stress that music therapy is not a standalone cure.

“Music therapy is not a replacement for conventional rehabilitation, but a scientifically grounded complement to it,” clarifies Dr. Sudhir Kumar.

Dr. Jindal agrees, emphasizing that while music therapy cannot replace vital physical or occupational therapies, it serves as an excellent adjunctive tool that also reduces post-stroke anxiety and depression — factors that frequently impede physical recovery.

Ultimately, this collaborative effort between engineers, neurologists, and speech therapists underlines the power of cross-disciplinary medicine. As Prof. Vibha summarizes, “We are using music as a tool to improve their language.” The ultimate objective is not to teach stroke survivors how to sing, but to help them regain their voice and reconnect with their loved ones.
