CORVALLIS, Ore. — For millions of adults who reach for cannabis to unwind at the end of a long day, the plant is often viewed as a reliable buffer against daily pressure. However, new research suggests that for frequent users, the body’s internal stress response system may tell a different story when the alarm rings in the morning.

A study conducted by researchers at Oregon State University found that individuals who use cannabis frequently wake up with significantly higher baseline levels of cortisol—the body’s principal stress hormone—compared to non-users. While the findings, published in the peer-reviewed journal Cannabis, do not prove that cannabis directly causes this morning surge, they add critical nuance to popular assumptions regarding cannabis and stress management.

What the Study Found

To evaluate how regular cannabis consumption interacts with daily hormonal rhythms, researchers led by Anita Cservenka, Ph.D., an associate professor in the School of Psychological Science at Oregon State University, recruited 82 adult participants. The sample was divided into two distinct cohorts: 39 frequent cannabis users and 43 non-using controls.

Participants collected saliva samples at three specific intervals across two weekdays:

  • Immediately upon awakening

  • 30 minutes post-awakening

  • In the evening

The research team specifically analyzed the cortisol awakening response (CAR)—a natural biological phenomenon characterized by a sharp rise in cortisol levels within the first hour of waking up, which helps prepare the mind and body for the demands of the upcoming day.

However, the primary divergence occurred at the starting line: frequent cannabis users exhibited markedly higher cortisol levels right at the moment of awakening. Furthermore, the study noted that higher baseline waking cortisol was correlated with a greater frequency of negative cannabis-related consequences in daily life.

Cortisol, produced by the adrenal glands via the hypothalamic-pituitary-adrenal (HPA) axis, plays a pivotal role in regulating essential bodily functions, including blood pressure, blood glucose levels, metabolic rate, and inflammatory responses.

“The findings do not establish cause and effect,” noted Dr. Cservenka in a release accompanying the study. “Long-term longitudinal research is needed to determine whether frequent cannabis use drives higher waking cortisol, or whether individuals possessing higher inherent stress biology are simply more likely to use cannabis frequently to self-medicate.”

Medical experts not affiliated with the trial point out several key limitations inherent to observational designs:

  • Sample Size and Demographics: The study examined a modest sample of 82 young adults, meaning results may not generalize across older demographics, adolescents, or patients utilizing medical cannabis under physician supervision.

  • Variable Product Dynamics: The study did not control for variations in product potency (THC to CBD ratios), total dosage, or administration methods (e.g., inhalation vs. oral ingestion).

  • The “Chicken-or-Egg” Dilemma: Pre-existing factors such as underlying anxiety disorders, sleep disturbances, lifestyle stressors, or subtle withdrawal symptoms during nighttime fasting could account for the morning spike rather than the direct action of cannabinoids on the HPA axis.

Clinical Implications and Practical Takeaways

For public health professionals and consumers alike, the investigation highlights a crucial message: regular cannabis consumption should not be viewed as biologically neutral regarding the body’s physiological stress architecture.

While many users report subjective relaxation following acute use, long-term frequent exposure may interact with the nervous system in unexpected ways. Healthcare providers are encouraged to integrate routine screenings for cannabis use frequency when evaluating patients presenting with refractory sleep disturbances, mood disorders, or chronic stress symptoms.

For consumers, key considerations include:

  • Avoid Assuming Universal Benefit: Cannabis is not a clinically validated long-term treatment for stress or anxiety disorders.

  • Monitor Behavioral Patterns: Individuals who require daily consumption to cope with routine stress or who notice worsening morning fatigue/anxiety should discuss these patterns with a primary care physician.

  • Prioritize Sleep Hygiene: Ensuring consistent sleep-wake cycles and evaluating non-pharmacological stress reduction techniques (such as cognitive behavioral approaches or mindfulness) remain foundational to long-term health.
