GENEVA — Contaminated food poses a far greater threat to global public health and economic stability than previously understood, causing an estimated 866 million illnesses and 1.5 million deaths annually, according to a landmark report released today by the World Health Organization (WHO). The comprehensive analysis, published June 4, 2026, in The Lancet Global Health, reveals a profound crisis of equity: children under five bear a wildly disproportionate share of the suffering, while low- and middle-income countries carry the vast majority of the global disease burden.

The updated estimates track 42 major foodborne hazards across 194 countries from 2000 to 2021. While overall global cases have trended downward since the turn of the century, the fresh data uncovers a dual-threat landscape. Biological hazards like bacteria and viruses cause the vast majority of acute illnesses, but chemical contaminants—predominantly heavy metals—have emerged as an unexpected, silent driver of mortality, accounting for nearly three-quarters of all foodborne deaths.

“Food safety is not an abstract issue – it touches every meal, every family, every day,” stated Dr. Tedros Adhanom Ghebreyesus, WHO Director-General, in a press briefing. “Until now we lacked the bigger picture of its staggering human and economic toll. These new estimates change that.”

The Vulnerable Shield: Children at Extreme Risk

The report’s most alarming finding centers on the extreme vulnerability of young children. Infants and toddlers under five years old make up just 9% of the global population, yet they account for nearly one-third of all foodborne illnesses. This means a young child faces nearly three times the risk of getting sick from contaminated food compared to an adult.

For young children, the dangers are two-fold:

  • Acute Hazards: Common diarrheal diseases caused by foodborne bacteria, viruses (such as rotavirus), and parasites can rapidly lead to severe dehydration and death in small bodies.

  • Chronic Hazards: Early exposure to toxic chemical hazards like lead and methylmercury can permanently damage a child’s developing brain, resulting in lifelong neurological deficits and learning disabilities.

“A young child’s immune system and gastrointestinal tract are still developing, making them highly susceptible to infections that an adult body might easily fight off,” explains Dr. Aris Papageorgiou, a pediatric infectious disease specialist at the London Institute of Global Health, who was not involved in the WHO study. “Furthermore, because children eat more food relative to their body weight than adults, their relative intake of heavy metals and toxins is significantly higher, amplifying the potential for long-term developmental harm.”

The Heavy Metal Toll: Toxins Driving Fatalities

While familiar pathogens like Salmonella or E. coli dominate headlines, the WHO report highlights a shifting paradigm: chemical exposures in food are remarkably lethal. In 2021, chemical hazards accounted for 73% of all deaths linked to contaminated food.

The primary culprits are inorganic arsenic and lead, which together caused more than 1 million deaths in a single year. These toxins gradually accumulate in the body, dramatically increasing the long-term risk of cardiovascular diseases, kidney damage, and various cancers.

Unlike bacteria, which can often be killed by proper cooking or pasteurization (heating liquids to kill harmful microbes), heavy metals enter the food chain through contaminated soil and water. Once absorbed by crops like rice or root vegetables, they are virtually impossible to remove.

A Severe Crisis of Regional Inequality

The data underscores a stark geographic divide. The African and South-East Asian regions together account for nearly 75% of all foodborne illnesses and 60% of global deaths.

This health burden translates into massive economic disruption. WHO estimates that in 2021, foodborne illnesses caused $310 billion in lost productivity due to time taken off work. When adjusted for international cost-of-living differences, that economic toll skyrockets to $647 billion.

Public health experts emphasize that these numbers are being exacerbated by modern environmental shifts.

“The data show that foodborne diseases are not only persistent but are being made worse by climate change, which increases contamination risks, and by antimicrobial resistance (superbugs that survive antibiotic treatment), which makes infections harder to treat,” said Yuki Minato, a WHO technical officer for food safety and senior author of the paper. Minato urged nations to adopt a “One Health” approach, which links human, animal, and environmental health to secure the food supply.

Limitations: The Hidden Burden

Despite the massive scope of this 21-year analysis, independent experts note that the true burden of foodborne illness is likely even higher. The WHO study was restricted to 42 specific hazards due to data gaps in many regions.

Crucial factors were excluded from the final tallies due to insufficient global data, including:

  • Pesticide residues

  • Per- and polyfluoroalkyl substances (PFAS, often called “forever chemicals”)

  • Antimicrobial-resistant bacteria

  • Miscarriages and stillbirths caused by Listeria infections

Public health agencies acknowledge that until localized food tracking and hospital reporting systems are strengthened in low-resource countries, millions of cases will continue to go unrecorded.

What This Means for Consumers

While large-scale policy changes—such as stricter industrial regulations and agricultural oversight—are required to curb chemical contamination, everyday consumers can take actionable steps to protect themselves and their families from biological hazards.

Separate Keep raw meat, poultry, and seafood completely segregated from ready-to-eat foods in your grocery cart and refrigerator to avoid cross-contamination.
Cook Use a food thermometer to ensure meats reach safe internal temperatures. Opt for pasteurized dairy products and juices, particularly for young children or pregnant individuals.
Chill Refrigerate perishable foods and leftovers within two hours of cooking (or one hour if ambient temperatures exceed 90°F / 32°C).

As World Food Safety Day approaches on June 7, under the theme “From burden to solutions – safe food everywhere,” public health officials hope this new data will serve as a definitive roadmap for governments to protect their most vulnerable citizens.
