A new directive from U.S. Defense Secretary Pete Hegseth requiring annual testosterone screening for all active-duty and reserve service members aged 30 and older has ignited a sharp debate among medical experts. While the Pentagon characterizes the policy as a proactive measure to bolster combat readiness, resilience, and performance, leading physicians are voicing significant concerns, arguing that the plan lacks robust scientific support and risks exposing healthy service members to unnecessary medical interventions.

Because testosterone levels naturally drop by roughly 1% per year after age 30, experts argue that routine screening in a young, active population may lead to the “medicalization” of the natural aging process.

Many clinicians advocate for a structured, evidence-based approach. Rather than universal screening, they suggest that resources would be better spent focusing on modifiable lifestyle factors, such as optimizing nutrition and sleep, and conducting targeted testing only when a service member presents with clear clinical symptoms like chronic fatigue, reduced muscle mass, or sexual dysfunction.

The Pentagon’s policy is viewed by some as an unprecedented, large-scale implementation of hormonal screening with little preliminary data to justify the potential downstream costs and health consequences. As the military moves forward, medical professionals are calling for greater transparency, clearer clinical criteria, and a shift toward evidence-based medicine that prioritizes long-term health outcomes over short-term performance metrics.
