NEW DELHI/CHANDIGARH — The overall burden of cancer across North India has escalated significantly over the last five years, driven by aging populations, environmental risk factors, and improving diagnostic detection. According to the latest estimates from the Indian Council of Medical Research’s National Cancer Registry Programme (ICMR-NCRP), tabled in Parliament by the Union Ministry of Health and Family Welfare, cancer cases across Punjab, Haryana, and Himachal Pradesh rose by nearly 10% between 2021 and 2025.

Among these northern states, Haryana recorded the steepest percentage spike at 11.26%, while Punjab continues to bear the region’s highest total volume of cases and mortalities. Nationally, India’s annual cancer trajectory expanded from 14.26 lakh estimated cases in 2021 to 15.70 lakh in 2025—representing a broader national surge of approximately 1.43 lakh new cases over five years.

Breakdown of Key Regional Findings

The data submitted to Parliament offers a stark statistical snapshot of how the oncological landscape is evolving across northern states:

  • Haryana: Recorded the sharpest relative growth in the region, climbing from 30,015 estimated cases in 2021 to 33,395 in 2025. This reflects an addition of 3,380 annual cases. In 2025 alone, Haryana witnessed an estimated 18,387 cancer-related deaths—translating to roughly 50 fatalities every day.

  • Punjab: Continues to lead the tri-state area in absolute disease burden. Cases rose from 39,521 in 2021 to 43,196 in 2025 (an increase of 3,675 cases). Punjab’s cancer mortality remains severe, with an estimated 24,886 deaths recorded in 2025, or approximately 68 deaths daily.

  • Himachal Pradesh: Reported the lowest overall numbers and percentage increase in the region, moving from 8,978 cases in 2021 to 9,761 in 2025 (an addition of 783 cases). Estimated deaths in the hill state reached 5,382 in 2025 (about 15 per day).

On a national scale, this annual increase translates to roughly 28,000 newly diagnosed cancer patients and more than 15,000 cancer-related deaths added to the country’s health baseline every year since 2021. States with larger populations, such as Uttar Pradesh (2.26 lakh new cases in 2025), Maharashtra, West Bengal, and Bihar, recorded the highest absolute totals nationwide.

Dissecting the Surge: Demographics, Environment, and Lifestyle

Medical specialists emphasize that rising case numbers are the result of a complex interplay between demographic shifts, enhanced surveillance, and genuine environmental and behavioral risk factors.

1. Demographic Shifts and Better Surveillance

“The observed trend is consistent with statistical projections and is largely driven by population growth and an aging demographic profile,” explained Dr. Mukurdipi Ray, a surgical oncologist at the All India Institute of Medical Sciences (AIIMS), New Delhi.

Dr. Ray noted that while India’s crude incidence rate remains stable at around 100 cases per 100,000 population, the overall volume creates an immense operational load on healthcare systems. He further cautioned that underreporting in rural sectors suggests the actual ground-level burden is likely higher than official registries capture.

2. Environmental and Occupational Exposures

Beyond expanding lifespans, environmental hazards are increasingly coming under scrutiny. A 2025 review cited by health authorities highlighted that the contamination of groundwater and surface water sources by industrial runoff, heavy metals, synthetic pesticides, and pharmaceutical pollutants is linked with an elevated incidence of gastrointestinal malignancies, particularly rectal and colorectal cancers.

In agrarian and industrial hubs across Haryana—such as Fatehabad, Sirsa, and Hisar—health reports and epidemiological reviews conducted by institutes like PGIMER, Chandigarh, have identified intense pesticide use, industrial emissions, and polluted water supplies as probable contributors alongside traditional lifestyle risks.

3. Lifestyle and Behavioral Drivers

Established modifiable risk factors remain a primary cause of cancer development across the population. High rates of tobacco usage, alcohol consumption, physical inactivity, poor dietary habits, rising obesity rates, and exposure to specific viral infections (such as Human Papillomavirus and Hepatitis B/C) continue to drive high incidence rates across both urban and rural demographics.

Regional Patterns and High-Burden Clusters

Sub-regional analyses reveal distinct localized trends across different districts:

  • Gender-Specific Trends in Haryana: Districts such as Hisar, Rohtak, Jhajjar, Gurugram, Jind, and Faridabad report disproportionately higher cancer incidence among men. Conversely, female cancer rates are markedly higher in Ambala, Rohtak, and Gurugram.

  • Cancer Typology: Head and neck cancers—frequently linked to tobacco consumption—remain predominant among men in Rohtak. In contrast, urban centers like Gurugram report higher instances of breast and prostate cancers. Cervical cancer remains a significant health challenge among women in districts like Ambala.

Public Health Infrastructure and Government Initiatives

To combat the rising numbers, the Union Ministry of Health and Family Welfare has expanded several preventive, diagnostic, and financial interventions:

  • Population-Level Screening: Under the National Health Mission (NHM), Ayushman Arogya Mandirs leverage frontline health workers—including Accredited Social Health Activists (ASHAs) and Auxiliary Nurse Midwives (ANMs)—to screen adults aged 30 and older for oral, breast, and cervical cancers.

  • Financial Protection: Cancer care coverage is integrated into the Ayushman Bharat–Pradhan Mantri Jan Arogya Yojana (AB-PMJAY), providing health insurance cover of ₹5 lakh per family annually for secondary and tertiary hospitalization across empanelled facilities.

  • Affordable Therapeutics: Initiatives such as the Pradhan Mantri Bhartiya Janaushadhi Pariyojana (PMBJP) and AMRIT pharmacies work to provide essential anti-cancer medications and surgical consumables at discounted rates.

  • Infrastructure Expansion: The central government is supporting the creation of State Cancer Institutes (SCIs) and Tertiary Care Cancer Centres (TCCCs). The 2025–26 Union Budget approved the establishment of Day Care Cancer Centres in district hospitals nationwide, alongside expanding network hubs supported by the Tata Memorial Centre in cities like New Chandigarh, Sangrur, Varanasi, and Muzaffarpur.

Persistent Challenges and Data Gaps

Despite policy efforts, systemic bottlenecks continue to hinder optimal cancer control. A major clinical challenge remains late-stage presentation: nearly 60% to 70% of cancer patients in India are diagnosed at advanced stages (Stage III or IV), drastically reducing five-year survival rates and increasing treatment costs.

Furthermore, healthcare experts highlight a critical shortage of trained surgical, medical, and radiation oncologists, as well as an uneven geographic distribution of radiotherapy machinery.

In states like Haryana, the absence of a comprehensive Population-Based Cancer Registry (PBCR) presents an operational hurdle. Without granular, real-time data tracking case stages, survival metrics, and treatment outcomes, allocating oncology infrastructure accurately across underserved rural districts remains a difficult task.

Practical Action: Prevention and Early Detection

For the general public, reducing cancer risk involves addressing modifiable lifestyle factors while participating in routine screening.

Primary Prevention Strategies

  1. Eliminate Tobacco: Avoid all forms of tobacco, including cigarettes, bidis, and smokeless chewing tobacco, which account for a substantial percentage of preventable head, neck, and lung cancers.

  2. Adopt a Nutritious Diet: Prioritize whole grains, legumes, fruits, and vegetables while limiting heavily processed foods, red meats, and excess alcohol.

  3. Maintain Physical Activity: Regular moderate exercise helps regulate body weight and lowers systemic inflammation associated with various malignancies.

  4. Environmental Safety: Use verified safe drinking water sources, particularly in regions with high agricultural or industrial runoff.

Recognizing Warning Signs

Individuals should consult a qualified medical professional if they experience any of the following persistent symptoms lasting more than two to three weeks:

  • Unexplained weight loss or chronic fatigue

  • Painless lumps or swelling in the breast, neck, armpits, or elsewhere

  • Non-healing sores or white/red patches inside the mouth

  • Noticeable changes in bowel or bladder habits

  • Unusual bleeding or discharge

Regular screening at local health centers can detect precancerous changes or early-stage lesions long before symptoms become severe, significantly improving the chances of successful treatment.
