New research from Edith Cowan University (ECU) in Australia suggests that Large Language Models (LLMs) like ChatGPT are acting as a “digital bridge” for those too intimidated to seek traditional face-to-face therapy. The study, which surveyed users who utilized AI for mental health support, indicates that the perceived anonymity and effectiveness of these tools are helping to dismantle the walls of anticipated stigma.
The Fear of Being Judged: Why We Turn to Bots
Mental health stigma is generally categorized into two forms: anticipated stigma (the fear that others will discriminate against you) and self-stigma (the internalization of negative stereotypes that leads to low self-esteem).
The ECU study, led by Clinical Psychology researcher Scott Hannah, found that users who viewed ChatGPT as an effective resource were significantly more likely to report a reduction in anticipated stigma. Essentially, because a computer does not “care” or “judge” in a human sense, users felt a level of safety they couldn’t find in a doctor’s office.
“The findings suggest that believing the tool is effective plays an important role in reducing concerns about external judgment,” Hannah noted.
For many, the act of typing a struggle into a chat box is a “low-stakes” entry point into the mental health system. It allows a person to practice articulating their feelings without the fear of a raised eyebrow or a clinical diagnosis being permanently etched into their record.
A Growing Trend: The “Accidental” Therapist
While OpenAI did not design ChatGPT to be a mental health professional, the public is using it as one anyway. This phenomenon—where a tool is adopted for a purpose outside its original scope—is known in technology as “emergent use.”
The ECU team found that despite the lack of clinical training, users are turning to AI for private, anonymous conversations. This is particularly prevalent among demographics who feel marginalized by traditional healthcare systems or those living in “mental health deserts” where practitioners are scarce.
Dr. Arshya Vahabzadeh, a psychiatrist and digital health expert not involved in the study, notes that this reflects a massive gap in current care models. “Patients often wait months for an appointment and then have to overcome the vulnerability of speaking to a stranger,” Vahabzadeh says. “An AI is available at 3:00 AM, costs almost nothing, and offers total privacy. From a patient’s perspective, the logic is clear.”
The Risks: Hallucinations and Ethical Gray Areas
Despite the potential to reduce stigma, the ECU researchers issued a stern warning: ChatGPT is not a doctor. The study highlighted that while users felt safer, the technical limitations of AI pose real-world risks.

The consensus among health experts is that AI should be viewed as a complementary tool rather than a replacement. The goal is to move users from the “safe harbor” of the chatbot into the “active care” of a professional.
By reducing the initial fear of judgment, AI can serve as a “pre-therapy” tool. If a user learns through a chatbot that their symptoms are common and treatable, they may feel empowered to finally book an appointment with a licensed psychologist.
“We need to understand how AI can safely complement mental health services,” the ECU team concluded. This might include AI tools specifically trained on clinical datasets that can triage patients or provide evidence-based Cognitive Behavioral Therapy (CBT) exercises.
