Ernakulam, Kerala – In a significant move to combat the rising tide of diabetes and hypertension, Kerala has been chosen as the testbed for the Indian Council of Medical Research’s (ICMR) ambitious DIGI-CARE project. The initiative aims to revolutionize the management of non-communicable diseases (NCDs) by integrating cutting-edge digital solutions into the state’s public healthcare system.

A Digital Lifeline Amidst a Health Crisis

Kerala, renowned for its high health standards and life expectancy, now faces a daunting paradox: surging rates of diabetes and hypertension threaten to undermine its public health achievements. According to the 2023 ICMR-INDIAB study, Kerala ranks third in India for diabetes prevalence (23.6%), trailing only Goa and Puducherry. Even more alarming, 44% of the state’s population suffers from hypertension-one of the highest rates in the country and significantly above the national averages.

Experts attribute this crisis to rapid urbanization, dietary shifts towards energy-dense foods, sedentary lifestyles, and high rates of abdominal obesity (39.5% in Kerala versus the national average of 28.6%). The coexistence of diabetes and hypertension is pushing Kerala towards a potential public health emergency.

DIGI-CARE: A New Approach to NCD Management

The DIGI-CARE project-short for DIGItal technologies for optimising the Continuum of CARE for Diabetes and Hypertension Management in the Public Healthcare System in India-is part of the National Health Research Priority Projects (NHRP). It is being simultaneously rolled out in five states and one Union Territory: Rajasthan, Assam, Chhattisgarh, Himachal Pradesh, Kerala, and Puducherry.

In Kerala, the project will be implemented in Ernakulam district in partnership with the Amrita Institute of Medical Sciences, Kochi. The initiative will unfold in several phases:

  • Assessment: Identifying current gaps in digital adoption within the healthcare system.

  • Co-Design: Collaborating with stakeholders to develop tailored digital interventions.

  • Implementation: Using Plan-Do-Study-Act (PDSA) cycles to test and refine solutions.

  • Evaluation: Measuring the effectiveness of digital tools in improving patient outcomes.

Key technologies include telemedicine platforms, mobile health applications, and digital workflow optimization-all designed to enhance early detection, patient engagement, and disease management.

Collaboration and Patient-Centric Care

The project is backed by close coordination between Kerala’s Additional Chief Secretary, the ICMR Director General, the Directorate of Health Services, the State Mission Directorate of the National Health Mission, and district-level health authorities. Emphasis will be placed on patient privacy, data confidentiality, and maintaining the highest ethical standards.

Officials stress that digital technologies can bridge critical gaps in care, especially at lower-level health facilities. By democratizing access to screening, monitoring, and specialist consultations, the project aims to set a replicable model for other regions grappling with similar NCD challenges.

A Model for the Nation

As Kerala grapples with its NCD epidemic, the DIGI-CARE project represents a proactive step towards modernizing public health management. If successful, the initiative could serve as a blueprint for other Indian states, demonstrating how digital innovation can address the complex challenges of lifestyle diseases in the 21st century.
