NEW DELHI — In a significant move toward achieving universal health coverage, the Government of India announced this week that the Pradhan Mantri Bhartiya Janaushadhi Pariyojana (PMBJP) has successfully established 17,990 Jan Aushadhi Kendras (JAKs) as of December 31, 2025. With a newly minted target to reach 25,000 outlets by March 2027, the initiative is transforming the country’s pharmaceutical landscape by providing high-quality generic medicines at costs 50% to 80% lower than branded alternatives.
The expansion comes at a critical time when out-of-pocket expenditure on healthcare remains a primary driver of poverty for millions of Indian households. By focusing on supply chain digitization and rigorous quality protocols, the program aims to dismantle the long-standing stigma surrounding generic drug efficacy while ensuring that life-saving treatments for chronic conditions like diabetes and cardiovascular disease remain affordable.
High Quality, Low Cost: The Price-Fixing Formula
At the heart of the PMBJP’s success is a transparent pricing strategy designed to undercut the market dominance of expensive brands. The Pharmaceuticals and Medical Devices Bureau of India (PMBI) employs a strict internal committee to review and fix the Maximum Retail Price (MRP) of the 2,110 medicines and 315 medical devices in its basket.
For essential medicines listed under the National List of Essential Medicines (NLEM), prices are capped at least 50% below the government-mandated ceiling price. For non-NLEM drugs, the price is set at least 50% lower than the average MRP of the top three leading brands for that specific product.
“The goal is not just to provide cheap medicine, but to provide value,” says Dr. Ananya Sharma, a public health policy expert not affiliated with the PMBI. “By anchoring the price to a significant discount off market leaders, the government is effectively forcing a competitive shift that benefits the consumer directly at the pharmacy counter.”
Ensuring Efficacy: The “Quality First” Mandate
One of the primary hurdles for generic adoption globally is the perception that lower cost equals lower quality. To counter this, the PMBJP has implemented a multi-layered quality assurance framework that exceeds standard retail requirements.
The mechanism relies on three “Golden Rules”:

WHO-GMP Certification: Only manufacturing plants certified for Good Manufacturing Practices by the Central Drugs Standard Control Organisation (CDSCO) are eligible to supply the scheme.

100% Pre-Testing: Every single batch of medicine arriving at the five national warehouses undergoes anonymized testing. No batch is dispatched to a local Kendra until it receives a passing grade.

NABL-Accredited Labs: Testing is conducted exclusively in laboratories accredited by the National Accreditation Board for Testing and Calibration Laboratories (NABL).
Solving the Supply Chain Puzzle
Expansion is meaningless if the shelves are empty. Recognizing past challenges with stockouts, the government has overhauled its logistics. Since September 2024, a new incentive model has encouraged JAK owners to maintain a steady stock of the 200 most commonly used medicines.
Furthermore, an end-to-end IT-enabled system now tracks demand in real-time. By monitoring 400 “fast-moving” products and using digital forecasting methods, the PMBI can predict surges in demand—such as anti-allergics during change of season or anti-infectives during monsoon—and adjust procurement accordingly.
Public Health Implications: Managing Chronic Disease
The PMBJP product basket is strategically curated to cover major therapeutic groups, including oncology, cardiology, and gastroenterology. For patients with chronic conditions, the savings are transformative.
Consider a patient with Type 2 diabetes. A monthly regimen of branded metformin and sitagliptin can cost upwards of ₹1,500. At a Jan Aushadhi Kendra, that same regimen may cost less than ₹400. Over a decade of treatment, these savings represent the difference between financial stability and debt.
“When patients can afford their medication, adherence rates skyrocket,” notes Dr. Rajesh Varma, a cardiologist based in Mumbai. “The greatest tragedy in medicine is a treatable disease going unmanaged because of a price tag. The expansion of these centers into rural districts is a vital step in preventive cardiology and oncology.”
Challenges and Counterarguments
Despite the growth, the program faces hurdles. Critics point out that while the number of centers is growing, the “last-mile” delivery in deep rural pockets still lags behind urban clusters. Additionally, some healthcare providers remain hesitant to prescribe generics, citing concerns over bioequivalence—though the government maintains that all PMBJP drugs meet stringent international standards.
To address this, the PMBI has launched “Jan Aushadhi Week” (the first week of March) and utilizes “Nukkad Nataks” (street plays) and WhatsApp chatbots to educate citizens in local dialects.
How to Access and Report Issues
The government has also emphasized accountability. If a consumer encounters overcharging or poor quality, they can utilize the Centralised Public Grievance Redress and Monitoring System (CPGRAMS) or call the dedicated helpline at 1800 180 8080.
As India marches toward its goal of 25,000 centers by 2027, the PMBJP stands as a blueprint for how state-led intervention can balance the scales between pharmaceutical profit and the fundamental right to health.
