Punjab Chief Minister Bhagwant Mann was recently diagnosed with leptospirosis, a rare bacterial infection that spreads to humans via the urine of infected animals, such as rats and dogs. The condition, caused by Leptospira bacteria, was confirmed by experts on Saturday. Dr. M Wali, Senior Consultant at Sir Ganga Ram Hospital, provided crucial insights into the disease and its potential dangers.

Understanding Leptospirosis: A Rare but Widespread Zoonosis

Leptospirosis is an infection caused by Leptospira, a spiral-shaped bacterium from the family Leptospiraceae. Dr. Wali explained that this bacterium, often transmitted through the urine of infected animals like rats, dogs, pigs, and even horses, typically enters the human body through contact with contaminated food or water. In rare cases, the disease may spread through unprotected sexual intercourse, particularly during anal sex.

The bacteria thrive in warm and moist environments, making tropical regions more susceptible to outbreaks. Despite being underreported, leptospirosis is endemic to several Indian states, including Gujarat, Kerala, Tamil Nadu, Maharashtra, Karnataka, and the Andaman and Nicobar Islands. The disease first emerged in India in the 1920s, with its earliest known outbreak traced to the Andaman Islands.

Symptoms and Diagnosis

Leptospirosis presents with flu-like symptoms, including high fever, headaches, muscle pain, vomiting, and diarrhea. Dr. Wali emphasized that eye involvement is common, with yellow or brown patches sometimes appearing. Additionally, abdominal pain can be a sign of the infection.

While the disease can resolve within two weeks with the help of antibiotics, such as doxycycline, severe cases may lead to complications affecting the kidneys, liver, brain, spinal cord, or lungs. Leptospirosis is notoriously difficult to diagnose due to its non-specific symptoms. A simple PCR test can help detect the infection, but many patients may go undiagnosed for extended periods, seeking treatment from multiple doctors before the condition is identified.

How Serious is Leptospirosis?

In most cases, leptospirosis is self-limiting and resolves with appropriate treatment. However, the disease can become life-threatening if it spreads to vital organs, particularly the brain. Although leptospirosis does not spread from person to person, it poses a significant risk through environmental contamination. Dr. Wali urged the public to avoid consuming loosely available food items from unsealed stores, as rat-infested groceries could serve as a source of infection.

Leptospirosis can have serious public health implications, particularly in countries like India, where it remains highly neglected. Despite causing significant mortality, the disease is underreported due to a lack of awareness among healthcare providers and the public, combined with inadequate diagnostic resources. In response, the Indian government launched the National Programme for Prevention and Control of Leptospirosis (NPPCL) in 2015, focusing on endemic states to curb the disease’s spread.
